Thank you for volunteering to do this home check for FAITH Animal Rescue. Please fill in the form below as honestly as possible.

Please fill in this form during the home check to ensure all relevant questions are asked as accurately as possible.

* Before starting the check please ask the homeowners permission to take photos of the garden and areas the cat or dog will live in. There is a photo upload button at the bottom of this form. Photos will only be used for reference by FAITH staff in the homecheck process.

 

You do not have to show the potential adopter the form or if the check has passed or failed, just tell them the form will be submitted and the Faith Staff will be in touch.
